- The Insurer (the company issuing you the policy)

Your policy number and the dates of when the policy is effective (and when it expires), in case you need to file a claim or reference your policy.
3. Property Information
This information will be different depending on what you are insuring.
For auto insurance, this will be the make and model of your vehicle, as well as the VIN number. If you’re purchasing homeowner’s insurance, this will be the property’s address.
4. Coverage Details
This section will make the bulk of your declarations page, so it’s important to read it carefully!
